Jersey City Medical Center, a Barnabas Health facility, was one of only six hospitals in the nation – and the only one in the New York metropolitan area – to be recognized by America’s Essential Hospitals for its outstanding work to improve quality and population health.
Jersey City Medical Center was recognized for its Wealth from Health: Patient Engagement Incentive Program.
It was one of two hospitals receiving Honorable Mention for the Gage Award, which recognizes activities that improve delivery, access, or value for specific populations and improve health outcomes.
“Jersey City Medical Center is focused on individuals living a healthier lifestyle,” said Joseph F. Scott, the hospital’s President/CEO.
Wealth from Health is an innovative reward points program that empowers patients to effectively manage their chronic health condition by tying reward points to completion of disease education models and care plan compliance. Research shows that costs and utilization for the emergency department and hospital fell based on a comparison of one year before the intervention and one year after.
America’s Essential Hospitals is the leading association and champion for hospitals and health systems dedicated to high-quality care for all, including the most vulnerable. Since 1981, America’s Essential Hospitals has initiated, advanced, and preserved programs and policies that help these hospitals ensure access to care. They support member hospitals with advocacy, policy development, research, and education.
The association announced its 2015 Gage Award winners at its annual conference, VITAL2015, in San Diego.
